Appointments to see a staff member may be made in person at Office 236 or Suite 245 Sargeant Student center or by telephone 218-281-8586 or 218-281-8585, voice mail or e-mail at or

If you are requesting an appointment for the first time (or first time this academic year):

  • come to 236 Sargeant Student Center to fill out the paperwork electronically and schedule an appointment


  • phone our reception desk (218-281-8586) to make an appointment
  • download the intake form (PDF)
  • fill out the information requested
  • bring the forms to 236 Sargeant Student Center at the time of your first appointment

Usually appointments can be scheduled within a day or two. Typically, counseling sessions will last 50 minutes and are scheduled on a regular basis as determined by you and your counselor. 

Students with concerns of an emergency nature can be seen immediately; staff members are also available for emergencies which may occur outside of regular office hours. Click on Staff link in left hand column of this page.

If you cannot keep an appointment, please notify us so we can use the time for another student. Please note that if you fail to show for more than one counseling session, or you postpone frequently, you will need to talk with your counselor by phone or email before being allowed to make another appointment.  Repeated cancellations or appointment no-shows will make you ineligible for our services.


Incident Reporting
Use this link to notify the University if you are a victim or witness of a crime on or off-campus. This form can also be used to report concerning or threatening behavior (e.g. talk about self-harm or suicide), scholastic dishonesty, discrimination, bullying, harassment, sexual assault or other violations of the University's Student Conduct Code.  Call 911 if you are in immediate danger or if you want to report the incident to the Crookston Police Department and/or Polk County Sheriff's Office.
